How to add a list of States and redirect based on Option chosen?

Hello, I'm working on an Accounting website and I'm trying to recreate this state list in squarespace, but I'm not sure how to do it. On the old website there is a list of states, you select a state and hit Go. Depending on the state you  choose, you will be redirected to that state's Tax page. Is there a way to do something similar in squarespace?

@tuanphan Yes, I would like something very similar to that, but I would like to chose the option and then have a button that I can click to redirect me.

Once the form block select field post-submit redirect change code is installed you do not need to edit it. All the action to configure it happens in the SS editor interface per the install instructions.

For the select field description you'd enter something like the following.

It might be possible but not with my code. My code basically ties into the functionality SS already provides.

I think you would need a new piece of code that overrides SS's behaviour and opened the URL in a new tab.

Hello I used your code and it was working fine, but I looked at it today and I'm not sure what is causing this to happen? I still have the drop down and Go button there, but it doesn't work now. Can it be from a url change or something?
