How to add an upload box to a form?

I'm looking for a way to add an upload file field to a form on a client's website. Is there a way of doing this?

You can use your existing Google Drive account.

You can connect any Google Drive account that you have access to.

Can I also connect it to a upload the forms into a specific Folder (or Subfolder within Google Drive? 

It goes into a subfolder, though I forget how much control you have over it's placement. You may have to give it a test and see how well it works for your needs.
