Form including Payment

Hi all, 

Looking for some ideas. Ive been tasked with recreating a website for a customer that will need a shopfront. Their shop though requires some custom functionality. The business sells advertising packages that asks the customer to fill out a comprehensive form and THEN take payment in that same form. So purchasing a digital product virtually, but allows the customer to fill out all necessary details. (See screenshots to see the form) 

The site URL above is NOT created on Squarespace, its on WIX at the moment... but I want to shift them to Squarespace. I need to ensure this type of thing can be done first. 

Can anyone tell me if this type of functionality is an option or not?

You cannot do this with Squarespace Forms, but you can use a third-party form like Jotform. They support comprehensive forms including multi-step forms as well as taking payment within the form. Note that these submissions/payments will not use Squarespace's Commerce system. They will be managed separately.
