Zapier Integration Not working

Hi - I am having trouble getting new leads to pull from my business account forms, specifically moving leads from "Squarespace" to "Lofty" (formerly Chime CRM). On Zapier, I have reset the tokens and re-synced with the Squarespace app for both apps.

I am able to get information out of Squarespace and inserted into a Gmail message. I am also able to sync leads from Facebook into Lofty (Chime).  So both apps are working, just not together. I can't figure out how to fix this.
