Embed Form Not Working

Hello. Using Squarespace version 7.0– Brine family (Polaris template). I am trying to embed a Breeze Church Management Software form onto my page.

When I use the Embed Block and try to add just the url https://authenticlife.breezechms.com/form/023c73 Squarespace says this is an invalid url. 

When I use either the Embed Block and input my own custom data or the Code Block, I use this code. <div class="breeze_form_embed" data-subdomain="authenticlife" data-address="023c73" data-width="100%" data-border_width="0" data-border_color="000000" data-background_color="ffffff" data-button_color="92b765"></div><script src="https://app.breezechms.com/js/form_embed.js"></script>

The problem I have here is when I'm on my home page https://authenticlifechurch.com/ and I use our top menu to navigate to the prayer page > hover over "connect" and then click "prayer", the form doesn't show up. If I hit refresh the form will show up. Why would this be? Is there a different code I can use to show the form without refreshing.

If you click "give" from the top navigation menu I have a different way of embedding a Breeze form but it doesn't look as nice and it auto drops down to the form when you click on the page. I don't want that either.

Would you know why our give page drops down to the form automatically after a few seconds? Is there a way to stop that? It's on this page https://authenticlifechurch.com/give.

@tuanphan let me know if my question doesn't make sense. Maybe when you click on the web link it doesn't jump down to the form for you.

Not sure. Try remove iframe code & check again.

If the problem disappear, the cause from iframe code ✌️
