Questionaire on homepage to lead you to direct results on website.

I'm looking to create a questionnaire/pop up form on the homepage so that the customers will be able to select what services they require, show the results from their choices, and then this form needs to notify a tradesman to contact the customer.

How can I do this or set up this automation?

This is a complex form/automation, and depending on the amount of questions/number of outcomes will require either an external service or custom-coded javascript form. This is something that could be added to a Sqauarespace website, but it is not possible* natively. I don't specifically know of a tool to do this.

*you may be able to set up a series of pages with question/answer buttons that lead you through a logic to a final page with a form to contact the correct tradesman, but this would be difficult to set up manage if you had more than a few questions. Effectively this is a flow chart with a page for every question.
