Intake forms

Hi there

I hope you are all well. Is it possible to have intake forms only required once per  account?

My client has a medical intake form and he only needs them to fill it once before booking an appointment, any further appointments booked using the same account do not need to fill it in again.

so far I haven’t seen an option for this.

At the moment, it’s turned on for every appointment.

You shouldn't transmit protected health information through Squarespace, except through the use of Squarespace Scheduling. Note that you can only make your Scheduling account HIPAA-enabled if you enter into a Business Associate Addendum (BAA) on the Powerhouse plan.

Squarespace Forms, such as Custom Product Forms and those created with the Form Block, are not suitable for the collection of secure patient information online. I recommend you use an external third party system designed for this purpose. 

Of course that might not be what you mean by "medical intake form" but without a link to the site, it's difficult to understand the context. We always recommend that you include a link to the site in your question.

This is through Squarespace Schedule, it’s for a fitness class.

Do you know if Squarespace schedule intake forms can do that?

A workaround is to create two different appointment types on Squarespace Scheduling. One for their first fitness class and one for subsequent fitness classes. You can then add a custom intake form on one appointment but not the other.
