Member input capability

Hi! 
I want to create a site where members can log in and have access to a series of questions. Members can answer those questions (privately) and return to view/edit their answers at anytime. They also need the ability to upload files. 

I'm trying to think of what this is similar to. 
- it's almost like a quiz, but not quite.
- seems aligned with learning management system for students
- i've seen this type of application in services like betterworks for employee self assessment
- almost like journaling  ¯\_(ツ)_/¯ 

Any thoughts on how to accomplish something like this in Squarespace?

You could create a Members Area and then embed a Google Form. Or just use a Google Form and send that out to members.
