help with file upload add-on

I have tried to add a "file upload form" to a page from File Request Pro and it won't go live.

I followed the Squarespace instructions and added the code correctly, why won't my Upload Page show???

I see it. Sometimes you can't see all changes done by code when logged in. You need to view logged out or from a second browser. I use edge to view changes and chrome to make changes to my site, I used to have two monitors which worked out great, drag from one side to the other, now I use two laptops one to edit, one to view.
