Forms and Functions in Squarespace

Hello!I have a client that needs an electronic signature at the end of a squarespace form for a waiver of liability. Anyone know how to do this? Or can direct me to code for something like this?

She also wanted her forms layed out and very custom, which was outside of squarespace capabilities... anyone know of a form add on or something that can be built outside of squarespace and then placed in via code?
