My product reviews are not displaying on the relevant product pages. Not sure how to fix it and get the reviews to display on items that customers have reviewed.

I would like to add a QR code to my Thank you cards that allow customers to write a review for their goods . Is it possible to direct them to the review form or is the only option that they get the email 14 days later ?
