Custom form not working

Hi all

im trying to add a pop up custom form to a product page, trouble is the apply function is unresponsive once form added, does anyone have a workaround. I am about to sign up with SS but need this function to work. Ive reported the issue and had various responses from SS of which none of them deal with my issue other than they are looking into it and it will take time to run tests, as this has been over 3 weeks now I am not sure what to do as this is 100% vital to my site.
