Specific form is opened based on submission of another form

Someone might have a better solution than what I am thinking, so I am going to say what I need and hopefully someone can help.

We have multiple services, and it would be nice if people could email to specific emails based on the service. Example: A spa website, where people could email to massage or mani/pedi or facial.

I would like a form or solution where the customer would choose the service, and based on their option it would open a form for the email (and then the email submission would be the service email).

I hope that makes sense, and I hope it's doable!
