Add a second button to a Simple List Item

Looking to add a second button to one of my list items in my Simple List. 

Currently looks like this:

I would like the right side entry to look like this, (and then be able to go in and change the link and description on the button):

Any thoughts?

Do you mean each item has a specific button (with a specific link) or all items have the same button (the same link at all button)?

You can add a text link in description, then we will give the code to turn link to button
