Storage options for forms

I am trying to update this site, which I've inherited.  I can't find a way to access or edit the storage options for the forms on the site.  One thread I found here suggested I'd get an tab when I try to edit the form that would say storage but I'm not getting that (I am 7.0 I think, brine template).

The form help mentions the kinds of storage options but doesn't tell me how to change it!

Can you take a screenshot of where and what you need to do? it help us easier to check your issue

Hey, thanks for answering so quickly.  Your anchor links worked great (other post).

Here are three screen shots.  I don't see a way to edit this form.  As I said, I didn't set this up but if I want to delete or change some of the fields, I don't how to do this.

I do get an edit tab but all I can do is look at the code.  See screen shots, per your request, below.

Also, is there any way to populate the request date field automatically?
