Safari cutting off form fields

Hi everyone, I've customised a form on my site slightly using CSS and it is appearing perfectly everywhere except for Safari on a 15inch screen. The form fields are getting cut off (see below image). 

All other browsers are fine and Safari on mobile and larger screens eg 27inch are fine it's just Safari on 15inch screen. This is the code I am using to customise.
