Custom adaptive form

I am having some trouble with the forms here. I am offering a service for people to send trading cards in for grading. They may send between 1 and 50 cards at a time. I need a form that asks for details about that many items. So if they order 5 it asks for 5 of the specified details or if they order 50 I need it to ask for 50 instead.

The specifics I need for each entry is Card Name, Card Set, Card Number, and Does This Card Have an Error or Special Mention You Would Like on the Label, ex. Swirl or Holo Bleed.
