The situation has been resolved. There was existing CSS code which was conflicting with the code you recommended.

The code is still active in the Custom CSS box.

The widget is sitting on the top lefthand side. When scrolling back up to the top, the dropdown fields sit in the header. Our contact at MindBody is also looking into this issue. Would you have any suggestions on how we can fix this? Thank you, Jess
