Adding an additional fee at checkout

I want to add personalisation to my candle labels at the checkout for customers to put their chosen words, names for an additional fee of £2. I know there is an additional form at checkout but how do I get it to include the £2 fee to the total order. Regards Kat

I can't seem to get my POP-UP to pop-up. I have gone through all the settings and still it wont appear. Any advice?
