Two Column Form Block

Hello, lovely community!

Can't get my head around how to display "MESSAGE" field on the right side instead of being it together with the rest of the list. May somebody help me? Attaching the reference of what I want to achieve (2nd screenshot).

Right Column: Message

Then button?

Do you use Personal or Business or Commerce Plan?

Add to Design > Custom CSS

Hi @tuanphan, I'm having similar issues with trying to create a 2 column form for ordering gift cards on my website. 

My website is www.liberateyourspacenyc.com with a. business account.

I would like to have the "your" fields on the left side, the 'recipient' fields' on the right side, and the how did you hear about us and tell us about your gift card to span the full width.
