Mobile Menu Not Working -- Misplaced code?

I loaded my website on my iPhone and found a red exclamation point in the place of the mobile menu bars. A squarespace rep noticed a floating box in the upper right corner of the site that is not part of the template code. He thought it could be injected code. On my iMac, the code displays a grey circle with "i" and when you click on it, an info page pops up that has our business info/map of location on it. Does anyone know how to delete this?
