Date format in form - change to DD-MM-YYYY

I was so frustrated that Squarespace Forms can only deal with dates in the North American format that I built a plugin to let you add a date-picker instead of typing the date.

You (the website owner) choose what format will be used to submit the date, for example you could have dd/mm/yyyy or mm-dd-yy or almost anything else, including days of the week.

There are several ways. How did you change the other labels? It's best to use the same technique.
