How to have conditional fields in a form block?

I am trying to create a form where i can give radio button but if one is selected, the visitor has to input another field as mandatory; that is one field is linked to another.

For example:
Where did you hear about us?

1. Website
2. Social Media
3. Other

And if “Other” is selected, the visitor must fill a text box “If other, kindly specify”.

How can I achieve this? I am trying to make a form for my Squarespace website. I have made a Text Block with placeholder text saying – “if other, kindly specify” but I cannot make it mandatory as it would be unnecessary if someone selects “website”.

Please help any direction would be great.

Rony I do not know if this can be done in SS with some code, but I have used Jotform for exactly this type of conditional functionality - the forms are easily embedded. check it out
