using footer to pass gclid in hidden form to crm

I am using code provided by google for the squarespace footer to pass GCLID in a hidden field on my form to my CRM. I think this is the area of code I am having problems with: 

var gclidFormFields = ['gclid_field', 'SQF_GCLID', 'SQF_GCLID_FIELD', 'sqf_gclid_field', 'sqf_gclid','TESTFORMGLCID', 'SQF_TESTFORMGLCID']; // all possible gclid form field ids here

I have created a test form with a hidden field called TESTFORMGCLID.

Any one have a suggestion as to what I am doing wrong?
