Help with clients Business tier page

a bit complex but anyone have any ideas how to pull this off

Business Tier page:

• This one is a little more complicated, this will be where users will purchase the business tier (the third tier for rental listers)
• Users that are looking to post more than one rental, can’t do it through the app and will have to instead do it through a form on the website/contacting us for specific pricing
• The form is basically the easier way for a user to purchase business tier instead of having to reach out to us they can do it through the website
• For simplicity sake I think the best way the form should be set up is as follows:

1. First name
2. Last name
3. Email used for their Shechuna account
4. How many rentals they want to post (each rental costing $12.99 per month)
5. A section that just displays text which says: that if they want to purchase boosts they can do it after directly through the app and if they want features they should contact us
6. When purchasing through the form (as opposed to through the app), users are not purchasing a subscription, rather a one-time purchase, so therefore: A simple calculation should be made like this:

• If the user pays only for one month then each rental is $12.99 (so if they were to be putting up 5 rentals they would be charged now $64.95)
• If the user pays now for 3 months each rental is $11.99 per month
• If the user pays now for 6 months each rental is $10.99 per month
• If the user pays now for one year (which will be the max) each rental is $9.99 per month

Let me know if that whole calculation thing makes sense

• So after filling out the form, they can then purchase it straight through the website
