I get error messages when testing the form on my site

I created a 'Speaker Inquiry Form' on my SQspace site using the "Add block." After publishing my site and testing the form by entering information, I got several error messages when I clicked the Submit button (please see attached screenshots). I don't understand why it's not accepting the information. Can anyone help? Thanks!

Oh wow! Yes, that must be it! I didn't realize there were all those field types to choose from. Duh! Sorry for the trouble, and thanks so much.:)

It appears you are using Email fields for everything. You need to choose the right field type for the kind of info you want to gather.

There are 19 field types to choose from.
