Constant Contact Forms - Not Submitting

Need some help with "Constant Contact" form installation. I have always used Squarespace native forms, but for years now I've had unsubscribers try to resubscribe to my emails and CC will not change their status and I'm missing out on lots of warm leads. They claim a solution would be to use a CC form and then returning sign-ups will be set to active in my contacts again. I've installed their "universal code" in CODE INJECTION > FOOTER and activated a "flyout form" which is global on the site after 5 secs. It presents correctly. However, you cannot submit the form. Button greys out and just hangs.

I've also tried installing their "inline form" with secondary code in on-page CODE BLOCK (in conjunction with the universal code) and same issue. I've left this form installed on a "not linked" page here: https://www.jodimurphy.net/home-apparel-samples

I also briefly installed it on another existing (very basic setup) 7.0 site and it seemed to work. I also tried switching the cookie banner from OPT-IN to OPT-IN & OUT with no success. 

Can anyone help identify what the issue or conflict with the code or script is on jodimurphy.net?

Can anyone tell if I have any conflicting code or scripts on the site? I have very little injected code. Is there any custom CSS that conflicts?

I see some code error appear when clicking submit button.

Can you send code in Code Block? I will test on my site easier.
