Create custom formula in a table/form

Hello,

I want to create a "game" on my Squarespace website where people will input certain information and it will calcuate the average cost of childcare. 

For example, in the form, a player adds the number of children (2), state you live in (U.S. states), type of care (private school), need transporation (yes/no), and need lunch (yes/no). Then, the player would select the button "Calculate your costs" and the total would appear. 

I have a CSV that has all of these numbers. 

So I'd like to be able to calculate 2 children, Illinois' private school ($700), transporation ($5), and lunch ($10). 

Is there a way to do this in squarespace?

@kendrac Yes, it can be done in Squarespace. However, it will need to be entirely built using custom code in HTML, CSS, and JavaScript.

Thanks @Lesum Is it super complicated? or something that can be done in a few hours/days?

@kendrac It's not complicated. But it would take couple of hours to complete.
