Hello fellow Squarespace designers! 

I need to add a few tables inside a form, and I don't see that as an option. Does anyone know how to do this? I'd assume I need to custom code within the form I've already started, but don't know how to do that. I'm attaching screenshots of the tables I need to add between standard form fields.

If someone can help me, I can provide the website password (it's not live yet).

Will the tables capture some data and pass it to the form or will they serve just for presentation ?

They are tables that the patient will need to fill out, to be submitted to the company along with the rest of the form. 

I think you can consider using lightbox, click a text inside form >> open a lightbox with form inside, if you want this, I can give you a quick guide.

I solved a similar case for a member a few days ago.
