Termageddon code affecting contact form

Hi everyone, i just got done contacting customer support and they redirected me here, really hope someone is able to assist. My contact form isn't working, and the customer service rep informed me that it works when they try disabling scripts, so that the issue is likely with some code that I've put in there. I used termagaddon to create the privacy policy/terms of service/disclaimer and that's what I have on my site. Would someone be able to help me identify what is causing the issue? I don't actually code so I'm a bit lost at this point. Thanks!
