donation form not working

the donation page isnt working for me nor for any of my clients. when the information form is completed is just says "Enter the required information.  " no matter how you answer. it was working a while ago before my site was having issues but now its back up and payments aren’t working. its been impossible to get support from squarespace as well..

Have you had any resolution to this issue? I am having the same in test mode.
