How to set-up one-off annual membership for a studen club

Hi,

I am customising a site for a student club, I need a way to set up a custom form and take payment in a single page/product, that will give members access to all the in-person benefits of the club. Do you have any ideas for solutions?

Setting it up as a digital service requires repeat subscription for the buyer, but because it's a student club we only want them to pay once.

I also tried setting it up as digital download, but that doesn't give me the ability to capture the member information.

The desired customer pathway is they scan QR code/follow a link to our sign up page > fill in the form > pay the one-off membership fee with the option to bundle merch.

Would appreciate any ideas and solutions.
