I need a solution where I can use a single form for all of my product pages.

IT needs to capture name, email, customization directions and upload graphics (and be capible of uploading multiple graphics.

I have a single form that can do this but need a universal one for my site.

Any sugestions?

If you've created a Custom Product Form, you can add this to all your products if required.
