Create a Quiz

Hi, 

I need to create a dual-answer quiz (i.e. one question can be answered either yes or no). Is this possible in Squarespace? And if not, are there any extensions that could make it possible?

You can try creating with use Form Block with Checkbox, Radio field...Or try Typeform
