Examples of ordering forms for a home based food business

Hi Everyone

I have a food business and was told by a customer that my order form is not that user friendly as its just a message box and they type in what they would like to order. 

He said try to setup so i have a list of all my menu items. He said I should make it easy so that the customer is able to check/select the item they want and a QTY box will appear on each item and they can select the quantity. 

I was hoping for some tips or examples form members here with that type of setup or something similar.
