How can I add a simple "was this useful" poll on squarespace?

I'd like to be able to track effectiveness of content. Simple Yes/No or thumbs up/down answer that I can track through google sheets or via analytics. 

Is this simple enough to code, a feature I'm missing on Squarespace or is there a recommendation for plugin?
