Searchable filters but more than standard allows

Trying to create multiple search categories for a church site for the volunteer teams. Similar to the image below. I can onlky figure out how to add category and tags but i need more. I want to create a signup button with a contact form to gather their information and pass it on the to proper team leader. Any tips or ideas?
