Contact link not working

At some point the contact page on my website stopped working.  I don't know how long it's been like this since I only found out when someone was very persistent in trying to get ahold of me.  I couldn't make it work so I deleted it and thought I'd start over.  This is where I'm stuck.  I add a new page called "contact" but I don't get a map that I think is supposed to pop up.  Also on the unlinked area below there is a contact with demo by it.  I clicked the demo on and nothing.  Ugh!

What map is supposed to be there? It sounds like you've selected a template contact page that has a different set up to what you had previously.

my apologies in not responding sooner.  My website is: www.rosemarycotnoir.com.  When I initially created the website on squarespace ...maybe 6 or more years ago if you clicked on the contact link a page came up that a interested individual could fill out their name etc. and write a comment and it would go directly to my email.  This is what stopped working.  For now I deleted the old form and just added a statement to send inquiries to my email address.

@rcotnoir Have you tried adding a new form block to the contact page and connecting it to your email address? If the old one had stopped working, this might be enough to 'refresh' it and get it going again
