How to add a link within a contact form

How can I go about adding a link to a terms of use page within a form block? I need users to be able to check that they've read and consent to the terms.

Hello! can someone please help me with this. I am working on a website through squarespace and I want to hyperlink something on my contact form. I'll attach a picture to make more sense of what I'm asking for. Does anyone know the CSS to do so? thank you so much!!!
