Regarding the technical capability, Squarespace has a form upload field built into Form Blocks. There's also a third-party add-on called the Squarewebsites Uploader (affiliate link). Regarding patient confidentiality/data protection, note that Squarespace is not compliant with many aspects of this so you may need to look into your client's legal requirements. There are third party solutions like Jotform that do have certain certifications. Did this help?
