Custom forms are not working

Hi,

I have tried on multiple products to create and enable custom forms on specific products. The issue I keep running into is when I add the item to my cart-the form pops up, quickly disappears before anyone can enter any info and nothing is added to the cart. I do not want to add this form to checkout as it is specific to only 4 products. The products are not currently on the store site since they will be sold starting in October. Any help would be greatly appreciated.

I was going to ask for a link to one of the products, but if they are not available for purchase, forum users won't be able to take a look and so won't be able to help. In the circumstances, I recommend that you remove any custom code that has been added to the site (and save it somewhere safe!) and then test again. If the issue persists (or if you don't have any custom code) please contact Squarespace Customer Care directly for assistance.

I did not put any custom code on the site, however, I cannot speak for the guy who initially designed it. Haven't seen any...but I'm not an expert either.
