Personalized URL (PURL) / Saving visitors / MySQL

Is there a product, plugin, or code example for squarespace that would allow me to have a Personalized QR code that opens a personalized link?   Then that website will save the fact that that particial purl was used.  

I'm not sure I fully understand, but there are lots of services that provide trackable QR codes.

Can you point me in the direction of some?

Im looking to have a printed QR code that has website.com?ref=FsdfgGfddfFD453  (i can generate that)

but then i was hoping, that if some one scaned the code, I could have my squarespace website  (in this case "website.com") pull that ref variable, and add the variable value to a mysql database....with perhaps a date and time stamp.

I don't have a specific recommendation, but any QR generator will be able to encode that URL, try googling and looking at the offerings:
