Dubsado Form Cutting Off

I hope someone can help - I have two contact forms one for portraits (which is working fine) and one for weddings, which is cutting off/has a scrolling function that I can't seem to get rid of. If I'm on the page and I refresh, it appears correctly but obviously first time visitors just assume my contact page doesn't work properly and they shouldn't have to refresh to have it work. Any suggestions?

I've tried duplicating the portraits page which is working properly, but when I input the code from Dubsado it does the same thing. Here is the code, I have not modified or tried modifying it.

Try Design > Site Styles > Disable Ajax Loading
