A pop up page asking for additional information

  If you take a minute and go on the website, Buy an Orientation package, this sheet pops up.

 

Well we don't need it and I can't figure out how to get rid of it.  And the bigger issue is at the bottom there is no way to submit this or get to the next page.  

*If you hover around the bottom of the page you will see "Add to cart."  

We aren't going to get anyone's business with this issue.  And even if you do click on it, it just empties the form and makes you do it all over again.

This is a Custom Product Form. If you want to remove it, edit the Product and then scroll down through the settings to Custom Forms and choose No Form Required to disable the Product Form.

This is a Custom Product Form.

If you want to remove it, edit the Product and then scroll down through the settings to Custom Forms and choose No Form Required to disable the Product Form.

If you want to keep the form and fix the button (which is currently set to white text and white border on a white background) you should be able to change the colours in the Site Styles panel. This button is a Primary button. Here's what it looked like when I changed the colours to black:

HI new problem.  Created a form for people to contact us.  The button says "Contact Us" and form are awesome.  Once again the button to submit their info does not show up.  I did what you said before and it is not working.  Is this not a Primary button?
