Contact Form Titles Within the Boxes

Hi! Is there a way to create the effect (see attached) where the titles of the contact forms are actually placeholders within the type boxes? Thanks!! 

Which page are you referring to? I don't see this on Homepage

I think we can use jQuery code to add these placeholder texts

Add this to Last Line in Code Injection > Footer

Can you take a screenshot all code in Code Injection Header + Footer after you added the code?
