Facebook ad conversions not showing up in squarespace or mailchimp

I am new to facebook ads so forgive me if this is confusing.  I have had a FB ad running since the December and it was working fine not converting great but logistically working.  On April 12th my leads started to increase and continued to increase until I was getting a 1.06 cost per lead....however the emails were not showing up in my mailchimp or squarespace account.  I have tried multiple things to resolve this but nothing seems to work.  Here is what I have done so far.

1.  Spoke with a meta business agent who assured me that nothing has been "hacked"

2.  Spoke with squarespace who reports maybe it is because I had opt in features turned on so turned them off.

3.  Deleted the form on my landing page and put in a new one

Any other ideas of why this would be happening.  I don't even really know where to start and I have "lost" a significant amount of leads due to this problem.  Do I have my pixels in the wrong place so FB is counting a "lead" as anyone who even visits my lading page?  Could it in fact be a FB problem?  Mailchimp?  I am so confused!!
