Header, Footer (and other content) disappeared with custom code to embed Dubsado form

Hi there! Hoping someone out there can help. 

I recently switched from Honeybook to Dubsado for my CRM. Previously, I had no issues with the Honeybook lead capture form being embedded on my contact page.

The Dubsado form has presented a couple of issues. First, it wasn't long enough, and only one field appeared at a time (with a scroll bar next to it). On Dubsado's advice, I changed the code to allow for 1000px height.

As soon as I did that, the content blocks next to the form on this page disappeared, as did the header and footer. 

I've since tried changing the code multiple times with different heights, and even tried removing the height portion of the coding altogether. Nothing brings back the header and footer. I moved around the other content blocks. Most of them appear ok now, except for the text block directly below the form. 

One more weird thing: if you start at the homepage (www.absleep.com) and click "Contact," the header and footer appear without issue. But if you go directly to the /contact-us slug, or refresh after arriving from the homepage, they disappear. 

What the heck have I done wrong? Many thanks in advance for your help! 🙂

Can you share link to page where you added form? We can check easier
