Add OpenTable widget to header?

Hoping to add an OpenTable widget to the header of a client's website. I've tried adding the code in the header code injection to no avail. 

I'd like for it to be where the Book a Table button is currently. Is this possible?

Also, when generating the code I'm given the option that is seen in an attached picture, should I keep it unchecked?

You mean when clicking button > Open OpenTable widget in a lightbox in current page?
