Creating Branching Questions in Form Block or Embedding a Survey

I am trying to create a form that has branching questions, e.g. will ask specific follow-up questions based on each form response. This is functionality I am told by Squarespace is not available as part of their Form block. I am wondering if anyone has been able to create form or survey questions that branch or if you have been able to integrate 3rd party applications for this use case.

This is going to be a third-party solution. You can do this with Google Forms and embed them with an embed block.
