Convert a PDF or doc to a form.

Is there any way to convert a .doc to a form?  Or, can I add a "submit" button to a text block, ie convert a text block to a form?

I am not a programmer so do not understand how to totally restyle a form.

A form is its own element, and you can't just create one in a text block and add a submit button. You can customize a form using CSS.
